Please check the boxes for days, times and sites you are available. If you are only available for part of a block of games, please list your available times or restrictions in the "Notes" field. The "Notes" field can also be used for any other information I need to know about (team/league conflicts, etc.). You can view the games as a list or in a calendar, although you will lose any checkmarks or other input when you switch between the two.
I will send a manual confirmation email within a day or so when I receive the submission. If you need to change or update your availability, email me directly or just resubmit this form with a note at the bottom.
